At Link Into Life, we are committed to providing exceptional, client centred support to people with disabilities, mental health issues, and aged care needs across Brisbane and Ipswich. Following our successful journey since 2019, we have officially moved into the Aged Care space and to support our expansion, we are seeking a motivated and experienced Support Coordinator/Care Coordinator to join our team.
About the Role; Now Hiring Support Coordinator
This is not just another role, it is a unique opportunity to drive the growth of our Aged Care services. While this position will initially involve supporting our established NDIS participant base, the primary focus will be on building and managing our Aged Care portfolio, ensuring our clients receive the highest quality of support to live independently.
Key Responsibilities:
Aged Care Growth: Managing a caseload of Aged Care clients, connecting them with community services and expanding our reach in the region.
Compliance: Ensuring all documentation meets both Aged Care Quality Standards and NDIS Quality and Safeguards Commission protocols.
Financial Management: Monitoring plan utilisation and budgets for both Aged Care and NDIS clients.
Team Collaboration: Working closely with our Director and team of Support Coordinators to streamline operations and share innovative ideas.
Who We Are Looking For
We need someone who is proactive, initiative taking and passionate about client outcomes.
Demonstrated experience in Aged Care, Care Coordination or NDIS, Support Coordination.
Certificate III in Individual Support (Ageing and Disability) and or relevant experience.
Strong knowledge of compliance and audit-ready processes.
High proficiency in Microsoft Office and clinical management software.
Excellent communication and problem-solving skills.
Must hold a valid driver’s license, reliable transport, and necessary screening checks (NDIS Worker Screening, Blue Card, and Police Check).
