Navigating supports and services can feel overwhelming, especially when you’re not sure where to start. At Link Into Life, we’re here to make the process clearer, easier, and more supportive from the very beginning.

We offer a free initial session designed to help you understand your options and take the first steps with confidence. Whether you’re new to the NDIS and want guidance on how to access it, need support preparing for your planning meeting, or would simply like to learn more about how our services can support you or your loved one, we’re here to help.

Support Coordination

Link Into Life can provide supports to assist you to make the most of your NDIS plan.
Some examples include:

  • Helping you understand whats in you plan

  • Finding services that fit with the kind of supports you are wanting

  • Work with other providers to ensure your needs are being met

  • Help get things ready for reviews.

  • Help work through issues that can come up with providers

  • Building your capacity to manage your supports

Specialist Support Coordination

This service is a time limited support and it is funded for when there is a high level of complexity in your plan.
Examples can include:

  • When you have Involvement with justice, Child Safety

  • Complex barriers that stop you accessing your supports

  • Building up your informal or other formal networks

  • When there is complex disability issues or mental illness

Therapeutic Supports – Social Work

As a Mental Health Social Worker, it is critical to ensure participants and their families are coping to the best that they can be. There are many things that Link Into Life can support you with through your NDIS funding:

  • Identifying solutions for current issues

  • Building emotional resilience

  • Career development – mapping out your goals and developing strategies.

  • Counseling around areas of anxiety, depression and other related disorders

  • Completing assessments, reports for your ongoing development and NDIS funding requirements

  • Grief and loss

  • Working through PTSD

  • Assisting with transitions including child into school, ageing or loss of independence

  • Explore stressors and build confidence to cope

  • Learning to manage news of diagnosis

  • Understanding changes in government departments such as Centrelink.

Life Stage Transitions

During your life there are different stages of time that can be difficult to navigate.  This could include educational, developmental, social or psychological/emotional. Link into Life can assist you:

  • Identify any roadblocks to your goals

  • Build your confidence and skills to go forward

  • Develop plans to help you cope

  • Link into services that can assist

Skill Development

As we progress in life sometimes we need help to build our skills.  Some supports Link Into Life can provide include:

  • Learn to budget, cook or clean

  • Build social skills

  • Public transport skills

  • Hobby development

Accommodation and Tenancy Support

Link Into Life can assist you with your accommodation and issues that arise.  This could include:

  • Helping to find accommodation

  • Talking with real estates or landlords on your behalf

  • Helping with lease agreements

  • Looking for financial support

  • Supporting with mediation or disputes that can arise

  • Write supporting documents
