Support Coordination is about people and services:


  • Helps you understand your plan and goals

  • Finds and connects you with providers

  • Sets up services and coordinates supports

  • Builds your confidence to manage your own plan over time

  • Prepares you for plan reviews and changes


A simple way to remember:


  • Plan management = handles the money.

  • Support coordination = handles the connections

  • Struggling to manage the budget and understand it, cannot find providers easily.

  • What could I do with that time if I weren’t managing the plan?

  • Would having a professional coordinator reduce my stress and improve my outcomes?
